A. Anyone can join. You don’t have to be an employee or part of a group. This plan is available to any resident of Florida.
Q. Who can be included as a dependent? A. Spouse, or dependent child under the age of 19 (under 25 if full-time student).
Q. What is the name of the plan? A. “Assurant Secure Choice” (underwritten by Union Security).
Q. How do I join? A. By fax (credit card only), by mail, or securely online.
Q. When wilI I be able to use the plan? A. Full benefits start on the 1st of the following month, if your application is received by the 15th of the current month.
Q. How long do I have to keep the plan? A. This is a one-year contract.
Q. What are the payment options? A. The only monthly payment option is bank draft. The draft will start for the 2nd month. The 1st month's premium, $35 set-up fee, AND a voided check are submitted with the application (you can also charge the 1st month and fee and submit a voided check). Monthly payments from a credit or debit card are NOT accepted. OR, pay the annual premium, plus $35 fee by check, money order, or credit card. Checks payable to "Union Security Insurance Co."
Q. How do I pick a Dentist? A. Click the “Dentist Directory” link above. Write the 7-digit Facility I.D. number of the General Dentist on the application. If we receive your application by the 15th of the current month, you will be listed on the roster at the Dentist's office on the first of the next month (it is not necessary to select specialists, like Orthodontists on the application).
Q. Will I get a card? A. Each family member will receive a card and benefit information about 3 weeks after we receive your application.
Q. How do I change my Dentist? A. Call customer service by the 15th of the current month to be on a different roster next month.
